Through our assistance, individuals are able to make their own decisions and choices to guide their own lives while working to achieve their goals and dreams. While the assistance required for each individual varies, typical services often include assistance with personal health care, activities of daily living, and prompting for appointments and upcoming activities.

Wellness Coordniation Tiers
Tier I

 Health care needs require at least weekly consultation/review with the RN/LPN including face-to-face meetings once per month.

Tier II

 Health care needs require at least weekly consultation/review with the RN/LPN including face-to-face meetings twice monthly.

Tier III

 Health care needs require at least twice weekly consultation/review with the RN/LPN including face-to-face meetings once per week.
